Yes it would be rather hard to get a doctor your best chance would be to fake passing out on a train hope a doctor was also there and present him with your problem it's about the only way without giving details.

It's a bit like that canoeist guy who faked his death with his wife helping I bet they prayed he wouldn't need a doctor not until they had at least sorted a fake id.
